How to Get a Record Request Form Completed
If a student would like us to complete a form that asks exclusively for disciplinary information, they can submit that form directly to our office. If the form asks for both disciplinary and academic information, it is typically submitted to a college services office. Staff in that college office can then work with us to complete the whole form.

Typically, background investigators will need to come to our office in person and present their identification along with a signed release from the student.
